Dually Involved Youth Program

CCN’s Dually Involved Youth Program facilitates the Wraparound process with youth that are involved with both the child welfare and criminal/legal systems.

How it Works

Through the Wraparound process, CCN’s Wraparound Facilitators coordinate and facilitate (or co-facilitate with the caseworker) monthly Family Team Meetings to collaboratively discuss needs, strategies, and ensure alignment on the plan of care amongst informal support systems (such as family, friends, and fictive kin), and the varying formal supports (such as caseworker, therapist, and service providers) working with the enrolled youth.

The team accesses resources and services through community-based resources and referrals or the use of flexible funding, when necessary.

The Wraparound process promotes stability and support, to:

  • Safely support youth within their communities

  • Prevent future criminal behavior

  • Help youth transition to adulthood

  • Develop a positive support team for the youth

This program’s Wraparound process can last up to two years.

Eligibility

 Aged 11-19 at enrollment

 In guardianship/custody of the Illinois Department of Children and Family Services (DCFS)

 Involved in the criminal/legal system

 From, and living in, the counties served (see below)

CCN’s Service Providers

CCN’s service providers for this program are OMNI, One Hope United, SGA Youth and Family Services, UCAN, and Youth Outreach Services.  

This program is funded and supported by Illinois DCFS.

Outcomes

Upon conclusion of our most recent fiscal year, CCN achieved the following success:

of CCN youth remain out of high-end care

(such as residential facilities and detention centers) during enrollment and in the 6 months after their closure from the program. This is compared to a baseline of about 35% of comparative youth that remain out of high end care, according to baseline data from DCFS.

allowing for their first Family Team Meeting to occur within a month of enrollment.
